Clinical Observation for Arterial Blood Gas Analysis and Pulmonary Function Test in Thoracic Surgical Patient


Abstract
Arterial blood gas analysis and pulmonary function test were done on 34 patients undergoing noncardiac thoracic surgery from June 1981 to April 1984 in the Department of Thoracic and Cardiovascular Surgery, Chung Ang University Hospital and Capital Armed Forces General Hospital.
Arterial blood gas analysis was done in 13 patients, they were analyzed by 3 groups such as pneumonectomy (group I), lobectomy (group II), miscellaneous operation (groupIII).
Pulmonary function test were done in 21 patients, they were analyzed by 3 groups, such as pulmonary resection(group A), decortication (group B), bullectomy (group C). The results are as follows:
1) The mean value of arterial blood gas on the first day following operation in pneumonectomy group was pH 7. 477, PCO2 32.9 mmHg, PO2 69.8 mmHg, oxygen saturation 93.8%.
2) The mean value of arterial blood gas on the first day following operation in lobectomy group was pH 7.479, PCO2 35.9 mmHg¢¥ P02 81.9 mmHg, oxygen saturartion 95.70/¢¥0.
3) The mean value of aterial blood gas on the first day following miscellaneous operation group was pH 7.482, PC02 36.2 mmHg, PO2 67.7 mmHg, oxygen saturation 94.50/,/.
4) The mean percentile change between preoperative and postoperative pulmonary function test in pulmonary resection group was 1516¢¥ decrease in vital capacity and 10% decrease in maximal breathing capacity.
5) The mean percentile change between preoperative and postoperative pulmonary function test in decortication group was 9% increase in vital capacity and 16;0 increase in maximal breathing capacity.
6) The mean percentile change between preoperative and postoperative pulmonary function test in bullectomy group was 18% increase in vital capacity and 19`%%0 increase in maximal breathing capacity.
